All children entering 7th grade need to have a checkup as well as being up to date on their shots. Especially the Tetanus. Make sure that you request a blue form for school, they will not admit your into school with out it! You may request on line or call the office, it takes 5 to 7 days to get the forms. Just a reminder if you had your child check up just email or call the office. If you haven't your check up this year and it was after May please check with your insurance company to see if they pay from the last check up or on a calendar year. We know your time is valuable and we do not want to come back for another visit or have to pay for the check up.

Its time for your child to register for Kindergarten. In order to make the process easy for you please schedule your appointment as soon as you can. If your childs last physical was after May last year check with your insurance company to see if they paid on a calendar year or from the last check up. We do not want to have to split the visit or have you pay for the check up. Next when you schedule the appointment make sure you tell that person that you will need your blue and gold forms! You will not be able to enroll your child with out these forms! If you had the check up but didn't get the forms call the office or request on line your forms ASAP it takes 5 to 7 days to get forms. This is a busy time for us and we do not want to make this stress for you and your child.
